Who is eligible for coverage?
Regular, full-time employees scheduled to work at least 30 hours per week are eligible for benefits. You must also be a citizen of the United States (U.S.) or non-U.S. citizen who is a legal permanent resident of the U.S.
Benefits are effective 1st of the month following 30 days of employment.
Are my dependents eligible?
Your eligible dependents include:
- Your legal spouse.
- A dependent child under age 26. Dependent children can remain covered until the end of the month in which they turn 26 for the Medical and Dental plans. Full-time student status is not required. If the dependent child is mentally or physically disabled, coverage can be extended beyond these age limits. Important Reminder: It is your responsibility to notify Human Resources after a change in eligibility for your spouse or child(ren) within 30 days.
- If an employee plans on covering a dependent (child or spouse) the employee must provide a copy of all dependents birth certificates, SSN card and marriage certificate.
What if my spouse also works for the Community Access Unlimited?
If you and your spouse are both employees of the company, only one of you may cover your dependent children. Additionally, you cannot be covered as both an employee and a dependent. You must each enroll.
When can I make changes to my benefits?
In most cases, your benefit elections remain in effect for the entire year (January 1—December 31). During each annual enrollment period you will have the opportunity to review your benefit elections and make changes for the coming year.
Family status changes include, but are not limited to:
- Marriage, divorce, or death of a spouse;
- Birth, adoption, placement for adoption, death, custody, reaching the dependent age limit, and marriage of a child or dependent;
- A change in your or your spouse’s employment that affects your benefits eligibility (starting a new job, leaving a job, starting or returning from an unpaid leave of absence or changing from part-time to full-time status, etc.);
- Change in your spouse’s benefit coverage with another employer that affects eligibility.
